Choose how you want to be seen
Book online any time through our scheduling system, call the office at (806) 698-8088, or walk in during clinic hours, Monday – Friday, 8:00 AM to 5:00 PM.
If leaving home is difficult for you or a loved one, our Mobile House Calls team provides primary care in private homes, assisted living facilities, and nursing homes. Call 806-74-HOUSE.

Save time by downloading our New Patient Paperwork. Simply download the forms, print and complete them, and bring them to your appointment.
The packet has five pages: patient information, consent to treatment, patient financial responsibility, consent for communication, and an alternative consent form for a minor accompanied by someone other than a parent.
What to bring
- Your current insurance card and any referral. Our financial policy asks for these before services are rendered at every visit. See Insurance and Payment for what is collected at your visit.
- Your completed new patient paperwork, if you filled it out at home.
Set up your patient portal
Once you are a patient, the portal lets you:
- Schedule new appointments
- Check upcoming appointments
- Send messages to our staff
- View current and past statements
- Update or review personal information
- Download and fill out paperwork
