Insurance and payment.

We would rather you know what to expect before you arrive. Here is how insurance and payment work at Lubbock Family Medicine, taken from our patient financial policy.

Do you accept my insurance?

Call (806) 698-8088 with the name of your plan and our staff will confirm your coverage before your visit. We honor all of our insurance contracts and take adjustments as instructed by our payers.

Medicare and house calls. House calls are covered in the same manner as a visit to your office-based doctor. If you normally pay $0 out of pocket to see your office-based doctor, you will pay $0 out of pocket for a house call, after any deductible is met. For most people who have Medicare and a Medigap supplement plan, the out-of-pocket cost is $0 after any deductible is met. Learn about Mobile House Calls.

Before your visit

Please bring your current insurance card and any referral to every visit. If you do not have them with you at the time of service, you will be asked to reschedule for a time when you can provide them.

At your visit

  • Deductibles, co-payments, co-insurance, and payment for any services not covered by your insurance are collected at the time of your visit.
  • We file your claim electronically when your insurance company accepts electronic submissions.
  • If your insurance company requires claims to be mailed, you pay for your visit in full at the time of service and we give you the codes you need to file for reimbursement yourself. Our receptionist will let you know before your visit if your plan has this requirement.

After your insurance pays

Any part of the balance your insurance leaves as your responsibility is due in full on or before your next visit. If you are unable to pay it by then, you will receive a letter giving you thirty days to pay the entire balance, including any charges from that day’s visit. Balances not paid in full within those thirty days result in dismissal from the practice.

We do not send paper statements. You will be told of any balance at your next appointment and given thirty days after that notice to pay it.

If you do not have insurance

Uninsured patients pay a $250 deposit when checking in with the receptionist. If your final balance at checkout is more than your deposit, the remaining balance is due at that time. If it is less, the difference is refunded to you at checkout.

Ways to pay

Transparency in Coverage

Machine-readable files published under the federal Transparency in Coverage rule are available here (opens in a new tab).

This page summarizes the Patient Financial Responsibility and Consent for Communication sections of our new patient paperwork (PDF). The signed paperwork is the controlling document.
